The challenges

  • Rural generalists cover the emergency department, inpatients and clinics across every specialty, often as the only doctor on site, with specialist advice by phone and telehealth cover that has competing demands.
  • Reception is often poor at remote sites and on the road between clinics, so guidance and records that need a connection cannot be relied on.
  • Overnight and on-call, nursing staff ask for decisions about patients the doctor has not seen, with little structured information to go on.
  • With shift patterns, handover from clinic staff about the previous day's patients is often limited, and the reasoning behind earlier decisions is lost.
  • Each health service and site has its own guidance, antibiograms and processes, and resistance patterns differ from metropolitan centres.

01 · LOCAL GUIDANCE

Guidance tailored to your region

Pathways incorporate the regional antibiogram, local resistance patterns and rural-specific advice where required. In the joint WACHS and UTS evaluation of the platform, participants identified local relevance as the key differentiator from national guidelines, and uptake was highest at the smallest sites. Patient-specific recommendations with clear escalation

Each pathway concludes with a specific agent, dose and duration, with dosing calculators integrated and the guideline version recorded. Endpoints indicate when to seek advice or arrange transfer.

03 · HANDOVER AND ON-CALL

A structured record of every nurse assessment

When nurses work through a pathway before calling, you receive the route they took, the answers given, the guidance version and any medicine supplied under protocol, so an on-call decision starts from structured information. The same records are waiting for review the next day, giving a reliable handover across shifts and sites.

Joint evaluation of the platform by the WA Country Health Service (WACHS) and the University of Technology Sydney (UTS), funded by the Digital Health Cooperative Research Centre (DHCRC-0248). Prescribing was compared before and after implementation in a balanced sample of three adopter and five non-adopter hospitals, and the estimates describe an effect observed where the platform was adopted.
